Measurements:

I agree to the following:

1. I verify that the above information given by me is correct.
2. The above measurements are mine and are correct, and I accept a size selection by Bella Bridal Gallery from the designer's sizing Chart.
3. That I will pay EXTRA for my alterations.
4. That if for any reason, such as gain/loss of weight, if I request the size, or if I become pregnant, I will pay above the normal alteration charges to correct any dress sizing problems, and Bella Bridal Gallery will not be responsible in any way for the dress sizing problems.
5. As a bride, I will be the point of contact between Bella Bridal Gallery & my entire bridal party, and will also be responsible for any unpaid balances for my entire bridal party, in the event of a wedding cancellation , or a bridesmaid drop-out, or for any other reason.
